Health declaration Form or the T.8 form is required for all nationalities without exception.
You will need to provide some personal information and some details about your planned stay in Thailand as well as some health information. It is necessary to have a confirmed plane ticket before submitting the form.
Please note that travelers are also required to provide proof of health insurance that covers COVID-related issues and to have a negative COVID-19 test (RT-PCR test) issued within 72 hours of departure.
